In 'Who banned you', write the name of the person who banned you.

In 'Your nickname', write the name you use on teamspeak 3.
In 'Reason for the ban', write the reason why you were banned.
In 'Unique ID', write your Unique ID (enter teamspeak 3 > Settings > Identities > Unique ID, you can find it in the bottom right).
In 'Screenshot (proof)', present a picture of the ban received.
Under 'Situation Description', please provide a brief description so we can resolve your request.
